Beyond Frozen Shoulder: 8 Causes of Persistent Shoulder Pain

Published on

Shoulder pain and stiffness can significantly impact daily life, making actions like dressing, lifting, or sleeping comfortably challenging. While frozen shoulder, or adhesive capsulitis, is a well-known cause of these symptoms, it is not the only condition responsible. Diagnosing the correct underlying issue is crucial, as various shoulder conditions necessitate distinct treatments.

Frozen shoulder occurs when the shoulder joint capsule thickens and tightens, leading to restricted movement and persistent pain. However, similar symptoms can arise from tendon injuries, arthritis, nerve issues, and other conditions. Identifying the differences between these conditions is essential. For instance, in frozen shoulder, both independent and assisted arm movements are limited, whereas a rotator cuff injury may only affect independent movement, with others able to move the arm further.

Primary frozen shoulder may develop without a clear trigger, often associated with middle age, diabetes, or thyroid disorders. In contrast, secondary frozen shoulder can follow an injury, surgery, or prolonged immobility, and may coincide with other joint or tendon issues.

Beyond frozen shoulder, persistent shoulder pain could result from a rotator cuff injury, shoulder arthritis, or calcific tendinitis, where calcium deposits cause sudden, intense pain. Nerve problems in the neck can also refer pain to the shoulder, accompanied by tingling or numbness. Other causes include labral injuries leading to shoulder instability, acromioclavicular joint pain, fractures or dislocations, and infections or inflammatory conditions that bring about severe symptoms like warmth, redness, and fever.

Medical advice should be sought if shoulder pain persists, disrupts sleep, or limits movement, as early evaluation can distinguish between joint stiffness and other causes. Immediate medical attention is necessary if shoulder pain follows a serious injury, or if there are signs of a medical emergency, such as chest pressure or breathlessness.

To protect shoulder health, avoid forcing painful movements or starting aggressive stretching before understanding the cause of symptoms. A balanced recovery plan, which may include activity modification, prescribed exercises, and pain management, is essential. An accurate diagnosis is the first step toward effective treatment, ensuring that persistent shoulder pain is not automatically attributed to frozen shoulder.
